Pashkow (the Cleveland Clinic Foundation) and Dafoe (U. of Ottawa Heart Institute) begin with four chapters on current perspectives, including one on evaluation of outcomes. The other five sections cover functional assessment and prescription of exercise; medical considerations, including rehabilitation of patients with pacemakers and common musculoskeletal problems in the exercising adult; psychosocial considerations, such as psychosocial interventions, vocational issues, and management of depression and anxiety; selected issues, including gender-specific issues and smoking cessation as a critical element of rehabilitation; and program implementation and function.
