In response to the death of Phoenix Sinclair, on March 21, 2006, the Minister of Family Services and Housing called for an independent review of Manitoba's child welfare system. This report represents one part of the review: the review the deaths of children and youth known to the child welfare system who died between 2003 and March 2006. The report presents summary information in terms of data, themes, observations of commonalities, trends and patterns.
